1. Get in touch
Starting the process with La Cura Support Coordination is easy. You can either email admin@lacuraqld.com.au or call 0450 855 815.
When you sign up for support coordination, it’s a good idea to have your NDIS plan handy, and if required, your nominee details.
Support coordination is funded through your NDIS plan under Capacity Building. Alternatively, you can use your Core Support budget to pay for a support coordinator.
2. Let's get to know each other
We will get in touch with you and chat about your plan, your goals and answer any of your questions. This is your chance to see if we are a good fit for you! If you want to move ahead, we will ask you to agree with a Service Agreement and sign a Privacy Consent Form.
3. Service Agreement
We will send you a copy of the Service Agreement for you to review and agree to. This document talks about how we will work together. We can go through the agreement with you and help you to complete your details in the Service Agreement form if you wish.
4. It's GO time!
We will meet with you at some place convenient for you – at your home, a café or over the phone if that’s more appropriate. Your La Cura Support Coordinator will go through your plan and make sure you understand what’s in it and identify the types of supports and products you could be using to make the most of your funds and achieve your goals. Then we get to work!
